Quality of life takes more than traditional treatment. Comprehensive and coordinated care systems are designed to integrate non-medical services and social support programs to answer to patient needs. Optimal care can be better achieved with integrated social services. In comprehensive and coordinated systems, teams of coordinators and social workers work closely with patients, caregivers and providing physicians to answer to your needs, whether medical or non-medical. Social workers who are uniquely qualified to work with Alzheimer’s, Multiple Sclerosis, Parkinson’s, and Stroke patients can help improve quality of life for patients, families, and caregivers.
Our licensed clinical social work programs coordinator Oylime Fernandez will provide the utmost service to our patients.
